Once you have made your initial appointment, you may pre-register by downloading, printing, and completing the forms below. Please be sure to bring them with you to your first visit. Completing the forms prior to your visit will help speed up the new patient registration process.

Clinic Policies

New Clients or Patients
Please arrive at least 15 minutes prior to your scheduled appointment. We are a medical spa, so we require all new clients or patients to fill out a skin and health history form. If you would like to complete these forms prior to your arrival, they are provided above for your convenience.

Standard Reservation Policy
A credit card number or gift certificate number is required to secure all new appointments and all medical service appointments.

Late Arrivals
Our scheduling is designed to allow for sufficient time for your service. In fairness to others, if you arrive late, your treatment will still end on time so the next client’s session can begin on time. With some treatments, a late arrival may require rescheduling your appointment. If you are more than 15 minutes late, it is considered a “no show” and you will be charged the $50 no show fee.

Cancellations and No Shows
Should you need to cancel or reschedule your appointment, please provide at least 24 hours’ notice to avoid a $50 no-show fee. All services cancelled or rescheduled the day of the appointment will be charged a no-show fee.

Product Returns
Your satisfaction is important to us. We accept unused and unopened products within 14 days with a receipt for a full refund. If you are not completely satisfied with a product purchase you have tried, please bring in the remaining product and receipt within 14 days. We will gladly exchange the full value of the product for product(s) more suited to you. No refunds or exchanges are provided after 14 days.

We accept cash, Visa, MasterCard, Discover, American Express, CareCredit, and Clara gift cards. We do not provide financing packages or payment plans. You can apply for CareCredit here.
